Currently Fountain Square is accepting WALK IN appointments! 

Location: 1429 Shelby St. Indianapolis, IN 46203

Time: Wednesdays and Thursdays

 Between 9:30am-11am and 2pm-3:30pm

What you need to know: 

Make sure you bring a valid photo I.D. and your medical insurance card.

Calling ahead often helps us quickly determine if we accept your medical or vision insurance. 

Scheduled appointments take priority. You may have to wait if you walk in. To avoid long wait times give us a call and we can schedule you!
